  PATIENT INFORMATION
 
 

 

Office Hours:  Monday through Thursday 8:00am to 4:45pm and Friday 8:00am to 4:30pm.    We are closed for lunch from 11:45 until 1:00.

Appointments:  Please call during office hours at (616) 301-2500 for appointments with Dr. Ramirez or Dr. Hoekstra. 

Cancellation policy:  If you need to cancel an appointment, please give us notice 24 hours in advance.

Billing Information:  For any billing questions, please call (616) 301-2500 and speak to our Billing Specialist.

Call Back Time:  We will try to return routine calls within 24 hours.  Urgent calls will be returned the same day.

Emergency and After-Hours Care:  Please call the main phone number (616) 301-2500 to reach the physician on-call.  They may direct you to be evaluated at a local med station or hospital emergency room.  For emergencies, please call 9-1-1 or go directly to the nearest emergency room.

Forms of Payment:  We accept cash, checks, and Visa or Mastercard (including debit   cards). Please note that payment of insurance co-pays is due at the time of visit.

Lab results:  Our policy is to call you with urgent results and send all normal or routine results to your home by mail or e-mail.  You will receive results within three (3) weeks.

Nurse Line:  If you have an urgent need to speak with someone in our office, please call the nurse at (616) 301-2500.  They can relay the information to your physician promptly, or give medical advice over the phone.

Parking:  We have plenty of parking available around our building within 100 feet of the front door ; our office is handicap-accessible.

Prescriptions:  Please call (616) 301-2500 to leave prescription refill requests.  Be sure to include the name and phone number of your pharmacy, your doctor's name, and the name (spell it) of the medication.  This process takes a few hours, so try to call a day or two in advance of running out.

Referrals:  Please contact our Referral Coordinator at (616) 301-2500 for information regarding any pending referrals or insurance authorizations for specialist services.  Organizing and finalizing this information may take up to 3-5 working days, depending on your insurance company.

Urgent Care Issues:  Please call the office at (616) 301-2500 and a triage nurse will assess your medical need and if medically necessary, you will be worked into a provider's schedule the same day.

Walk-Ins:  We will try our best to accommodate walk-in patients.  However, the best way to see your own doctor is by scheduling an appointment.  Please remember to bring all of your current insurance information.
